
What is GUIDE?

According to the 2025 data from the , an estimated 7.2 million Americans age 65 and older are living with Alzheimer鈥檚 disease or other dementias. Nearly 12 million Americans provide unpaid care for a family member or friend with dementia. GUIDE (Guiding an Improved Dementia Experience) is a program designed to improve dementia care for patients and their caregivers.
Dementia impacts individuals and their families. Most care is provided by family and friends who aim to keep their loved ones at home for as long as possible. However, the health and well-being of caregivers may decline as they face challenges in delivering care and oversight. The GUIDE program is here to help.
GUIDE has three main goals:

Improve the quality of life for people with dementia

Provide support for caregivers

Help people with dementia live at home and communities longer
The GUIDE program is available to assist, offer knowledge, experience, resources, encouragement, and ongoing support to individuals living with different stages and types of dementia, including their families and caregivers.
The Aging Brain Care Program and Madsen Geriatrics Clinic at the 亚洲自慰视频 of Utah are excited to offer the GUIDE program starting July 1, 2025.
Who is GUIDE For?
GUIDE is for people with dementia and their caregivers. A caregiver is a relative or unpaid family member who helps care for someone with a health condition. We recognize that not everyone with dementia has someone they consider to be their caregiver. You are still eligible to participate without a caregiver.
People with dementia (and/or their family members) can enroll and participate in GUIDE if they meet the eligibility criteria:

Have dementia
-
Live in Salt Lake County
-
Enrolled in Medicare Parts A and B
-
(Note: patients enrolled in Medicare Advantage, including Special Needs Plans and PACE are not eligible.)

May be dually eligible for both Medicare and Medicaid

Live at home or in an assisted living facility
Medicare benefits will not change when you choose to participate in GUIDE. Participants still have the right to use any doctor, health care professional, or hospital that accepts Medicare. There are no additional fees or copays to participate in the GUIDE program.

Our GUIDE team is committed to assisting individuals with dementia and their caregivers, ensuring they feel supported and prepared for their journey. When provided with information and support, those living with dementia and their caregivers can concentrate more effectively on what matters most.
We provide a variety of services:

Annual comprehensive dementia assessment with a specialized medical provider

Monthly check-ins (phone calls) with a GUIDE team member

Person-centered care plans to help guide care based on goals, needs, and what matters most to the person with dementia and their caregivers

Regular medication reviews

24/7 caregiver support

Ongoing caregiver education

Coordination with community resources and support services

Transitional care support through changes in health

In-home safety assessments for eligible participants

Respite services for eligible participants
